I understand that it is a policy of Eastern Area Agency on Aging (EAAA) for all staff and volunteers to wear a photo badge produced by EAAA. It is the responsibility of the staff/volunteer to be accountable for the badge. Should the badge be lost, it is the responsibility of the staff/volunteer to contact Human Resources at (207) 941-2865 to report a lost badge and to request to have another one produced. I understand that the badge is the property of EAAA and I agree to forfeit the badge at the point in which I have completed my assignment(s).
