I acknowledge that I have received and have read this Handbook. I have met with my supervisor or Manager and have had an opportunity to clarify any questions which I may have concerning one or more portions of this Handbook. I understand that a signed copy of this Acknowledgment will be kept in my volunteer personnel file. I understand that it describes the Agency's policies and procedures, and that it is a guide to assist volunteers in following these policies and procedures.
